Rules FAQ:

  • If another copy of a unique character in the chosen opponent's dead pile is already in play, either owned or controlled by you or by the chosen opponent, that character cannot enter play, neither as a character nor as a duplicate, and so it remains in the dead pile (see FAQ).

  • If there are two or more copies, by title, of the same unique character in the opponent's dead pile, House of the Undying will not put any of them into play.

  • Characters directly moved from play to the dead pile are not considered to be killed (similar to Heads on Spikes).

  • The owner of the cards (not the current controller) chooses in which order they are placed in the dead pile (relevant for Ghosts of Harrenhal).

Yes. Cards always go to their owner's out-of-play areas (not their controller's), and when a card leaves play, all lasting and delayed effects affecting this card expire for it. See RRG entries for "Ownership and Control" and "Leaves Play". — mplain 238
